1 / How do I know what type of screening I need?
More often than not your company/DER/consortium or any ordering official will either email you a confirmation page or give you a hardcopy in hand to bring with you when you come for your visit. it is imperative that we know the exact details of what kind of screwing is required. Without this information, we will be unable to fulfill your request.
2 / What do I need to bring to my appointment?
The first thing you will need to bring to the appointment is a valid form of identification. This can be a drivers license a passport or state issued ID. Next you must bring your confirmation letter in hand or make sure any email sent to you or forwarded to us. We will not be able to fulfill a screening without the precise information needed to perform which specific service is needed. Make sure you bring a chain of custody, If provided to you. Lastly, show up with the full bladder, ready to provide a sample once on site. If it is a hair follicle test, do not shave your head. This will not help you in anyway.
3 / How do I know if it is a DOT screening?
How do I know if it is a screening? A typical DOT screening will be for those in possession of a commercial drivers license or anyone with a safety sensitive possession in the workplace. Again your ordering official should make it clear via paperwork which test is needed to fulfill your screening.
